Recreational Bathing Facility License Application
This application is to license your Recreational Bathing Facility (Pool/Spa/Spray Park), below is a list of requirements for this application and information on the fees associated with this application. You will be required to fill out an application for each pool, spa, or spray park you may own and operate. If you have multiple pools, spas, or spray parks, each one will require a separate application. The necessary fees must be submitted with your application(s).
When completing this application, please make sure to:
· Fill out all the fields with the most recent and accurate information for your Recreational Bathing Facility
· You will be required to fill out all parts of this application even if there were no changes since the previous licensing season
· All facilities are required to submit a pre-operational checklist
· Pool/Wading Pool/Spa Drain Cover Expiration Dates
· Attach receipts for the Drain Covers to this application, if drain covers have been replaced since last year
· Drain Cover Replacement form must be filled out
· All Aquatic Plans shall stipulate actions to be taken for chemical and microbiological exceedances
· The Designated Adult Supervisor at pools >2000 sq. ft. shall possess a Pool Director Certificate, please attach that to this application if this applies to your facility
Fees:
$200.00 pool/per year
$200.00 spa/per year
There is no fee for wading pools or spray parks
You can now pay by credit card or e-check. For credit cards a fee of 2.65% of the total amount due with a minimum fee of $1.50 will be assessed and paid directly to the credit card processor (this is not refundable). To pay by eCheck there is a flat fee of $1.50 paid directly to the check processor (this is not refundable). If you prefer to send a check for the fee, it can be made payable to South Brunswick Township and mailed to South Brunswick Township, Health Department - Licensing, P.O. Box 190, Monmouth Junction, NJ 08852. Please note your application will NOT be processed until we receive the online payment, a check or payment in the mail or at the counter in the Health Department.
