Food Handling License Application
This license is in compliance with the South Brunswick Township Code Chapter 18- Board of Health Regulations under Sections 18-251 and 18-252. Read and understand the Township Code regarding Retail Food Establishments. Before you apply for a Food Handling License, does your food establishment fall under any of the following:
-new construction
-change in existing facility, equipment, floor plan, or menu
-change in ownership
-change in trade name (business name/signage)
If any of these apply to you, please contact the Health Department at 732-329-4000, extension 7237, so that we can go over the necessary requirements. Please DO NOT fill out an application until you have spoken with one of our Health Inspectors.
When completing this application, please make sure to:
-fill out all the fields with the most recent and accurate information for your food establishment.
-attach a copy of your food establishment’s current food menu
-attach a copy of your food establishment’s most recent Certified Food Manager Certificate.
-indicate the type of food service for your food establishment
-please select the correct fees that fit the criteria for your food establishment
You can now pay by credit card or e-check. For credit cards a fee of 2.65% of the total amount due with a minimum fee of $1.50 will be assessed and paid directly to the credit card processor (this is not refundable). To pay by eCheck there is a flat fee of $1.50 paid directly to the check processor (this is not refundable). If you prefer to send a check for the fee, it can be made payable to South Brunswick Township and mailed to South Brunswick Township, Health Department - Licensing, P.O. Box 190, Monmouth Junction, NJ 08852. Please note your application will NOT be processed until we receive the online payment, a check or payment in the mail or at the counter in the Health Department.
